Varsity Application
Each school that wishes to gain Varsity Membership, and play in the Varsity tier must complete this form. Only one submission required per school.
Whitelist Application
For non-Varsity programs - this form is to gain permission for full spectator access for all Matches during the Competition (some restrictions apply).
